Shift Type: Per diem availability; shifts vary by assignment. Typical options include Monday-Sunday, 7:00 AM - 3:00 PM (Day), 3:00 PM - 11:00 PM (Evening), and 11:00 PM - 7:00 AM (Night); weekends as needed

Roles & Responsibilities
- Provide compassionate direct patient care, focusing on safety and dignity
- Administer medications and treatments accurately and document thoroughly
- Perform routine assessments and communicate changes to the care team
- Assist with activities of daily living and promote patient independence
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Maintain accurate charting and complete required paperwork in a timely manner
- Support a positive environment for residents and families through clear communication and empathy
Qualifications
- Current LPN license in good standing (New York state licensure required or compact-equivalent where applicable)
- Current CPR/BLS certification
- Recent clinical experience in long-term care, skilled nursing, or similar settings preferred
- Up-to-date immunizations and ability to pass required health screenings
- Willingness to work per diem schedule and flexibility to cover varied shifts
- Clean background check and ability to meet facility onboarding requirements
Education
- Licensed Practical Nurse (LPN) diploma or certificate; active NY LPN licensure required
Knowledge & Skills
- Strong clinical judgment and attention to detail in patient care
- Medication administration and documentation best practices
- Ability to perform basic assessments and communicate findings clearly
- Compassionate resident-centered care and excellent interpersonal skills
- Comfort with electronic medical records and charting systems
- Teamwork, time management, and adaptability in a fast-paced environment
