Roles & Responsibilities

• Provide compassionate direct patient care, focusing on safety and dignity
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments and communicate changes to the care team
• Assist with activities of daily living and promote patient independence
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain accurate charting and complete required paperwork in a timely manner
• Support a positive environment for residents and families through clear communication and empathy

Qualifications

• Current LPN license in good standing (New York state licensure required or compact-equivalent where applicable)
• Current CPR/BLS certification
• Recent clinical experience in long-term care, skilled nursing, or similar settings preferred
• Up-to-date immunizations and ability to pass required health screenings
• Willingness to work per diem schedule and flexibility to cover varied shifts
• Clean background check and ability to meet facility onboarding requirements

Education

• Licensed Practical Nurse (LPN) diploma or certificate; active NY LPN licensure required

Knowledge & Skills

• Strong clinical judgment and attention to detail in patient care
• Medication administration and documentation best practices
• Ability to perform basic assessments and communicate findings clearly
• Compassionate resident-centered care and excellent interpersonal skills
• Comfort with electronic medical records and charting systems
• Teamwork, time management, and adaptability in a fast-paced environment
