Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
- Administer medications and treatments accurately and on schedule
- Perform routine assessments and communicate changes in condition to the care team
- Document care thoroughly and timely in the resident record
- Collaborate with nurses, CNAs, therapists, and families to deliver individualized care plans
- Support infection control practices and maintain a clean, safe environment
- Participate in shift handoffs and relay important clinical information clearly
- Assist with admissions, discharges, and transfers as needed to ensure continuity of care
Qualifications
- Current LPN license (License/Certificate Verification required)
- Basic Life Support (BLS) certification required
- Up-to-date immunizations and health screenings as listed by employer (PPD/IGRA, COVID-19, influenza, MMR, Hep B, Varicella, Tdap)
- Successful completion of Trinity 10 panel drug screen and required health documentation
- Fit test clearance and any facility-specific health questionnaires completed
- Comfortable working in a per diem pool with variable schedules
Education
- Graduation from an accredited LPN/LVN program or equivalent vocational nursing education
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration and documentation accuracy
- Effective communication with residents, families, and interdisciplinary teams
- Time management and ability to prioritize in a fast-paced long-term care setting
- Familiarity with infection prevention and resident safety protocols
- Professionalism, reliability, and adaptability for floating between units or shifts
