Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
• Administer medications and treatments accurately and on schedule
• Perform routine assessments and communicate changes in condition to the care team
• Document care thoroughly and timely in the resident record
• Collaborate with nurses, CNAs, therapists, and families to deliver individualized care plans
• Support infection control practices and maintain a clean, safe environment
• Participate in shift handoffs and relay important clinical information clearly
• Assist with admissions, discharges, and transfers as needed to ensure continuity of care

Qualifications

• Current LPN license (License/Certificate Verification required)
• Basic Life Support (BLS) certification required
• Up-to-date immunizations and health screenings as listed by employer (PPD/IGRA, COVID-19, influenza, MMR, Hep B, Varicella, Tdap)
• Successful completion of Trinity 10 panel drug screen and required health documentation
• Fit test clearance and any facility-specific health questionnaires completed
• Comfortable working in a per diem pool with variable schedules

Education

• Graduation from an accredited LPN/LVN program or equivalent vocational nursing education

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Medication administration and documentation accuracy
• Effective communication with residents, families, and interdisciplinary teams
• Time management and ability to prioritize in a fast-paced long-term care setting
• Familiarity with infection prevention and resident safety protocols
• Professionalism, reliability, and adaptability for floating between units or shifts
