Roles & Responsibilities
- Provide compassionate direct patient care, including personal hygiene and basic nursing tasks
- Administer medications and treatments according to scope and facility policy
- Monitor patient status and report changes promptly to the care team
- Document patient observations and care activities clearly and accurately in the chart
- Collaborate with RNs, providers, and support staff to deliver coordinated care
- Assist with admissions, discharges, and transfers to ensure smooth transitions
- Follow infection control and safety protocols to protect patients and staff
Qualifications
- Current Licensed Practical Nurse (LPN) license (state licensure required)
- BLS certification required
- ACLS certification required
- Completion of facility-required health screenings and immunizations (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Successful completion of Trinity specialty competency testing and skills checklist as required
- Clear Trinity 10 panel drug screen and license/certificate verification
Education
- Graduate of an accredited Practical Nursing program or equivalent training
- Current LPN license
Knowledge & Skills
- Solid clinical judgment in routine nursing assessments and basic interventions
- Medication preparation and safe administration practices
- Accurate and timely documentation and charting skills
- Strong communication and teamwork with patients, families, and interdisciplinary staff
- Infection control, safety practices, and attention to detail
- Comfort with facility competency testing and adherence to policy
