Roles & Responsibilities
- Provide compassionate direct nursing care to older adults enrolled in Mercy LIFE programs
- Administer medications and treatments safely and document per policy
- Monitor and record vital signs, changes in condition, and clinical observations
- Collaborate with nurses, social workers, and interdisciplinary team members to support care plans
- Complete Trinity Specialty competency testing and skills checklists as required
- Maintain accurate, timely electronic and paper documentation
- Support infection prevention practices and ensure immunization compliance
- Participate in orientation and ongoing professional development activities
Qualifications
- Current, unrestricted LPN license (practical nurse) — Pennsylvania licensure required or ability to obtain
- BLS certification required
- ACLS certification required
- Successful completion of Trinity Skills Checklist and specialty competency testing
- License/certificate verification and Trinity 10-panel drug screen required
- Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza (per facility policy)
- Completed required health screening: 2-step PPD or IGRA (Quantiferon) and physical/health statement
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma/certificate
Knowledge & Skills
- Strong clinical assessment skills and comfort with vital sign monitoring
- Medication administration and documentation best practices
- Familiarity with infection control and immunization protocols
- Ability to follow Trinity specialty competency processes and checklists
- Clear verbal and written communication for interdisciplinary teamwork
- Basic electronic health record (EHR) documentation skills
- Professionalism, reliability, and patient-centered care mindset
