Roles & Responsibilities

• Provide compassionate direct nursing care to older adults enrolled in Mercy LIFE programs
• Administer medications and treatments safely and document per policy
• Monitor and record vital signs, changes in condition, and clinical observations
• Collaborate with nurses, social workers, and interdisciplinary team members to support care plans
• Complete Trinity Specialty competency testing and skills checklists as required
• Maintain accurate, timely electronic and paper documentation
• Support infection prevention practices and ensure immunization compliance
• Participate in orientation and ongoing professional development activities

Qualifications

• Current, unrestricted LPN license (practical nurse) — Pennsylvania licensure required or ability to obtain
• BLS certification required
• ACLS certification required
• Successful completion of Trinity Skills Checklist and specialty competency testing
• License/certificate verification and Trinity 10-panel drug screen required
• Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza (per facility policy)
• Completed required health screening: 2-step PPD or IGRA (Quantiferon) and physical/health statement

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma/certificate

Knowledge & Skills

• Strong clinical assessment skills and comfort with vital sign monitoring
• Medication administration and documentation best practices
• Familiarity with infection control and immunization protocols
• Ability to follow Trinity specialty competency processes and checklists
• Clear verbal and written communication for interdisciplinary teamwork
• Basic electronic health record (EHR) documentation skills
• Professionalism, reliability, and patient-centered care mindset
