Roles & Responsibilities

• Evaluate patients’ mobility, strength and functional needs and craft individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve function and independence
• Document patient progress clearly and timely to support continuity of care
• Collaborate with nurses, physicians and the rehab team to coordinate holistic care
• Educate patients and families on safe mobility, home programs and fall prevention
• Adapt treatments to meet patient goals and organizational guidelines while maintaining safety
• Participate in discharge planning and recommendations to ensure safe transitions

Qualifications

• Valid Physical Therapist license in California or eligibility to obtain one quickly
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Clinical experience in acute, outpatient, or skilled nursing settings preferred
• Strong documentation skills and familiarity with electronic medical records
• Excellent communication and teamwork skills; patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered where applicable

Knowledge & Skills

• Skilled in therapeutic exercise, manual techniques and mobility training
• Knowledge of functional assessment and outcome measures
• Ability to develop progressive home exercise programs and education plans
• Familiarity with assistive devices, gait training and safe transfer techniques
• Strong clinical reasoning, time management and prioritization abilities
