Roles & Responsibilities

• Provide patient-centered physical therapy assessment and treatment to maximize functional outcomes
• Develop individualized treatment plans and document progress clearly and professionally
• Perform therapeutic interventions including mobility training, manual therapy, and therapeutic exercise
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Educate patients and families on home programs, safety and prevention to support long-term recovery
• Participate in discharge planning and communicate recommendations for ongoing care

Qualifications

• Licensed Physical Therapist in California or eligible for California licensure
• Current CPR/BLS certification as required by facility
• Comfortable with documentation in an electronic medical record
• Strong clinical assessment and therapeutic skills in adult/pediatric populations as applicable
• Excellent communication and teamwork skills; compassionate and patient-focused

Education

• Degree in Physical Therapy (Bachelor’s or Doctor of Physical Therapy preferred) or equivalent clinical training

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise prescription, and manual techniques
• Knowledgeable about mobility, gait training, balance and functional restoration
• Proficient in clinical documentation and care planning
• Ability to work effectively within an interdisciplinary team and adapt to site protocols
• Familiarity with safe patient handling and fall prevention strategies
