Roles & Responsibilities
- Assess and treat adults with communication and swallowing disorders in a skilled nursing facility
- Develop individualized treatment plans that drive measurable functional gains
- Collaborate closely with nursing, therapy colleagues, and families to coordinate care
- Provide clear, timely documentation and progress reporting to support continuity of care
- Implement evidence-based dysphagia management and rehabilitation strategies
- Participate in interdisciplinary care planning and discharge planning to optimize outcomes
Qualifications
- Current state license to practice as a Speech-Language Pathologist (Michigan licensure preferred)
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
- Experience in Skilled Nursing Facility (SNF) care or post-acute/rehab settings
- Strong dysphagia evaluation and treatment skills
- Excellent documentation skills and familiarity with regulatory requirements in long-term care
- Reliable, professional, and patient-focused with strong interpersonal skills
Education
- Master's degree in Speech-Language Pathology (or equivalent graduate degree) required
- Clinical fellowship completion or relevant clinical experience preferred
Knowledge & Skills
- Skilled in bedside swallowing evaluations, MBS/FEES familiarity a plus
- Knowledge of cognitive-communication assessments and therapy approaches
- Comfortable with fast-paced caseloads and prioritizing patient needs
- Proficient clinical documentation and electronic medical record use
- Collaborative communication with interdisciplinary teams and family members
- Ability to adapt evidence-based practices to the SNF population
