Roles & Responsibilities

• Assess and treat adults with communication and swallowing disorders in a skilled nursing facility
• Develop individualized treatment plans that drive measurable functional gains
• Collaborate closely with nursing, therapy colleagues, and families to coordinate care
• Provide clear, timely documentation and progress reporting to support continuity of care
• Implement evidence-based dysphagia management and rehabilitation strategies
• Participate in interdisciplinary care planning and discharge planning to optimize outcomes

Qualifications

• Current state license to practice as a Speech-Language Pathologist (Michigan licensure preferred)
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
• Experience in Skilled Nursing Facility (SNF) care or post-acute/rehab settings
• Strong dysphagia evaluation and treatment skills
• Excellent documentation skills and familiarity with regulatory requirements in long-term care
• Reliable, professional, and patient-focused with strong interpersonal skills

Education

• Master's degree in Speech-Language Pathology (or equivalent graduate degree) required
• Clinical fellowship completion or relevant clinical experience preferred

Knowledge & Skills

• Skilled in bedside swallowing evaluations, MBS/FEES familiarity a plus
• Knowledge of cognitive-communication assessments and therapy approaches
• Comfortable with fast-paced caseloads and prioritizing patient needs
• Proficient clinical documentation and electronic medical record use
• Collaborative communication with interdisciplinary teams and family members
• Ability to adapt evidence-based practices to the SNF population
