Roles & Responsibilities
- Provide compassionate, patient-centered home health nursing care to adults and families in their homes
- Perform skilled assessments, develop and implement individualized care plans, and document clinical findings clearly
- Administer medications, wound care, IV therapy and other skilled nursing treatments as ordered
- Educate patients and caregivers on disease management, medication use and safety to promote independence
- Communicate effectively with interdisciplinary team members and report changes in patient status promptly
- Coordinate visits and travel routes to maximize efficiency while maintaining high-quality care
- Maintain accurate, timely documentation to meet agency and regulatory standards
Qualifications
- Current, unrestricted Registered Nurse (RN) license in Vermont or compact RN licensure where applicable
- Home health nursing experience preferred; new to home health clinicians are welcome with relevant acute care experience
- BLS certification required; ACLS or specialty certifications a plus depending on patient population
- Comfort with independent decision making in the home setting and strong clinical assessment skills
- Reliable transportation and ability to travel between patient homes during shift
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Strong assessment and critical thinking skills in a home health environment
- Skilled in wound care, medication administration and IV therapy as applicable
- Clear, compassionate patient and caregiver education abilities
- Familiarity with home health documentation systems and regulatory requirements
- Time management and route planning for efficient patient visits
