Roles & Responsibilities

• Provide skilled occupational therapy services to home health and hospice patients, focusing on functional independence and safety
• Plan, implement, and document individualized therapy programs in accordance with Home Health/Hospice standards
• Perform OASIS assessments with accuracy and attention to regulatory requirements
• Conduct medication review and safety education when you are the only discipline in the home
• Coordinate care with families, managers, and interdisciplinary teams to ensure seamless service delivery
• Focus on geriatric needs and deliver compassionate support for patients and caregivers

Qualifications

• Licensed Occupational Therapist in North Dakota (required)
• Recent travel experience (required)
• Approximately 2 years of OT experience preferred
• Experience with OASIS and home health documentation practices
• Competency in medication review and knowledge of home health regulations
• Cerner experience strongly preferred
• Submission packet must include resume, relevant skills checklist, and proof of licensure

Knowledge & Skills

• Skilled in Home Health clinical documentation and regulatory compliance
• Proficient with OASIS-driven assessment and documentation
• Comfortable performing medication review and safety planning
• Experience with hospice care principles and family-centered education
• Familiarity with Cerner electronic health record is a plus
• Strong clinical reasoning and geriatric-focused rehabilitative skills
