Roles & Responsibilities

• Evaluate patients to identify impairments, functional limitations, and goals
• Develop individualized treatment plans focused on restoring mobility and independence
• Deliver evidence-based outpatient physical therapy interventions including sports and orthopedic rehab
• Provide targeted care for pediatric, neurological, and stroke patients when indicated
• Perform balance, dizziness, and wheelchair seating assessments and interventions
• Document progress clearly and collaborate with the interdisciplinary team to optimize outcomes
• Educate patients and families on home exercise programs and injury prevention

Qualifications

• Current unencumbered Oregon Physical Therapy license (required)
• Healthcare Provider Level BLS (required)
• Ability to travel to multiple points of care during the workweek (required)
• Experience in outpatient physical therapy; sports, orthopedic, pediatric, neurological, or stroke rehab preferred
• Comfort with balance/dizziness rehab, wheelchair seating, and neck, back, and extremity care

Knowledge & Skills

• Strong patient evaluation and treatment planning skills
• Experience restoring mobility and functional independence in outpatient settings
• Skilled in therapeutic exercise, manual therapy, and balance interventions
• Knowledge of documentation standards and outcome measurement
• Ability to adapt treatment for a range of diagnoses: ortho, neuro, pediatrics, vestibular
• Good communication and teamwork when coordinating care across sites
