Roles & Responsibilities
- Provide rehabilitative services to home health and hospice patients with compassion and clinical excellence
- Assess patient functional status, safety risks, and family needs to create individualized treatment plans
- Implement treatment and preventative procedures under medical direction to improve patient outcomes
- Coordinate therapy services with interdisciplinary teams and community resources
- Educate patients and families on maintenance, safety, and best practices for home care
- Document care thoroughly and communicate progress to supervisors and care teams
Qualifications
- Licensed Physical Therapist in North Dakota (required)
- Minimum ~2 years of prior Physical Therapist experience strongly preferred
- Recent travel experience required for this travel assignment
- Current driver’s license and reliable vehicle to travel to patient homes
- Cerner experience strongly preferred
- Submission packet required: resume, appropriate skills checklist, and proof of licensure (provide full NURSYS if RN; include licensure proof as requested)
- Must be located outside the 50-mile local radius (locals not accepted per client rule)
Knowledge & Skills
- Home health and hospice care principles, with emphasis on geriatric population needs
- Patient assessment, treatment planning, and rehabilitation techniques
- Strong documentation and clinical communication skills
- Ability to work independently in patient homes and adapt treatment plans to home environments
- Familiarity with Cerner electronic health records preferred
- Community resource knowledge and ability to consult/coordinate referrals
