Roles & Responsibilities

• Provide patient-centered physical therapy evaluation and treatment for residents in a long term care setting
• Develop individualized treatment plans that promote mobility, safety, and independence
• Perform therapeutic exercises, gait training, balance and fall prevention interventions
• Collaborate with nursing and interdisciplinary team members to coordinate high-quality care
• Document progress, update care plans, and communicate outcomes clearly and timely
• Educate residents and families on home programs and safe mobility strategies
• Participate in facility initiatives to enhance functional outcomes and veteran quality of life

Qualifications

• Current or compact Physical Therapist license in Montana, or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) preferred; relevant graduate-level PT degree accepted
• Basic Life Support (BLS) certification from an accredited provider
• Experience in geriatric care, skilled nursing, or long term care preferred
• Strong clinical reasoning, documentation skills, and a compassionate approach to veteran care
• Up-to-date immunizations and ability to comply with facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy basics, and functional mobility training
• Experience with fall risk assessment, balance interventions, and adaptive equipment recommendations
• Comfortable with clinical documentation and electronic health records
• Excellent communication and teamwork skills for collaborating with interdisciplinary staff
• Ability to educate residents and family members with clarity and patience
• Time management skills to balance caseload and maintain quality documentation
