Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living (bathing, dressing, toileting) to promote resident dignity
• Monitor and record vital signs and changes in condition, communicating concerns to the nursing team
• Support safe mobility and transfers, using proper body mechanics and assistive devices
• Assist with meal service and documentation of intake, respecting dietary needs and preferences
• Maintain accurate and timely documentation in the resident chart and handoff reports
• Collaborate with nurses, therapists, and support staff to create a consistent, person-centered care plan

Qualifications

• Active Licensed Nursing Assistant (LNA) credential in good standing
• Travel LNA (T-LNA) designation or willingness to complete travel documentation
• Current BLS/CPR certification preferred or ability to obtain prior to start
• Ability to pass background check, drug screen, and meet facility health requirements
• Comfortable working in long-term care/rehabilitation settings and adapting to new teams
• Strong communication skills and a positive, team-first attitude

Education

• Completion of an approved LNA training program
• High school diploma or equivalent

Knowledge & Skills

• Skilled in resident care, ADLs, and safe patient handling
• Familiarity with vital signs, basic charting, and reporting changes to nursing staff
• Good time management and ability to prioritize tasks in a fast-paced environment
• Strong observational skills and attention to detail for resident safety
• Compassionate bedside manner and ability to build rapport with residents and families
