Roles & Responsibilities
- Provide skilled physical therapy evaluations and develop individualized home health treatment plans
- Deliver hands-on therapy and progressive interventions to improve mobility, safety, and function in the home
- Educate patients and caregivers on safe transfers, fall prevention, and home exercise programs
- Document patient progress accurately and timely in the Home Care Home Base charting system
- Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
- Participate in orientation and required training to maintain clinical competencies
Qualifications
- Valid Physical Therapist license (ability to submit license required)
- Minimum 1 year of clinical experience (home health experience preferred)
- Current BLS certification (required)
- Comfortable working independently in patients' homes and managing caseloads
- Willingness to complete 12 hours of orientation as part of onboarding
Knowledge & Skills
- Strong knowledge of home health therapy principles and home safety assessment
- Skilled in mobility, gait training, balance interventions, and fall prevention strategies
- Proficient documentation skills, experience with Home Care Home Base is a plus
- Excellent patient and caregiver education and communication skills
- Ability to collaborate effectively with nurses, case managers, and other clinicians
