Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families, helping them manage diabetes and improve outcomes
• Utilize the nursing process to assess, plan, implement and evaluate individualized patient care plans
• Administer prescribed medications and treatments safely and in accordance with policies and state practice acts
• Document patient care and education accurately and timely according to standards of care
• Collaborate with the multidisciplinary diabetes management team to coordinate longitudinal care
• Act as a clinical resource and educator for staff on patient education programs and materials
• Participate in department performance improvement activities and shared governance to advance patient care quality
• Advocate for patients by addressing clinical and psychosocial issues and escalating concerns appropriately

Qualifications

• Licensed as an RN in Illinois and Wisconsin (required)
• Graduate of an accredited nursing program (BSN preferred)
• CDCES (Certified Diabetes Care and Education Specialist) required
• Approved CPR certification required within 90 days of hire
• Comfortable floating to various units and providing culturally sensitive, holistic care
• Availability for variable shifts, including potential weekends, holidays, and on-call coverage

Education

• Graduate of an accredited nursing program
• BSN preferred
