Roles & Responsibilities

• Provide skilled physical therapy evaluations and evidence-based treatments for residents in a skilled nursing setting
• Develop and implement individualized treatment plans that improve mobility, function, and quality of life
• Document assessments, treatment plans, and progress notes accurately and timely to support continuity of care
• Collaborate with nurses, OT, SLP, and interdisciplinary team members to coordinate patient-centered care
• Educate residents and families on recovery goals, safe mobility strategies, and home exercise programs
• Monitor and modify interventions based on patient response, safety, and evolving clinical needs

Qualifications

• PT or DPT credential (DPT listed among required skills)
• Comfort working in a Skilled Nursing Facility (SNF) environment
• Any experience level accepted — new grads are welcome and encouraged to apply
• Ability to work day shifts and follow facility policies; state licensure as required by the employer

Education

• DPT (Doctor of Physical Therapy) — listed in job skills

Knowledge & Skills

• Skilled in geriatric-focused physical therapy interventions and mobility training
• Strong documentation and clinical reasoning skills
• Ability to collaborate effectively within an interdisciplinary care team
• Knowledge of fall prevention, transfers, gait training, and adaptive equipment
• Professional communication with residents, families, and staff
