Roles & Responsibilities

Evaluate patients' functional status and rehab needs and create individualized treatment plans
Deliver compassionate, evidence-based physical therapy treatments focused on recovery and quality of life
Document patient progress clearly and promptly to support continuity of care
Collaborate with nursing and interdisciplinary teams to coordinate patient-centered care
Educate patients and families on therapeutic exercises, fall prevention, and safe mobility
Participate in discharge planning and transitions to ensure safe, successful outcomes

Qualifications

Current California Physical Therapy license (required)
CPR/BLS certification (required or obtained upon hire)
Experience in skilled nursing or post-acute care is preferred
Strong interpersonal and communication skills; patient-focused mindset
Ability to pass background checks and meet facility onboarding requirements

Education

Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy

Knowledge & Skills

Skilled in therapeutic exercise, balance and gait training, and transfers
Knowledge of documentation standards and rehab outcome tools
Comfortable working within an interdisciplinary care team
Good clinical reasoning and the ability to modify plans based on patient response
