Roles & Responsibilities

• Assess and evaluate patients to create individualized, evidence-based treatment plans
• Deliver skilled physical therapy interventions to maximize mobility, independence, and quality of life
• Document patient progress clearly and timely, ensuring regulatory and payer requirements are met
• Collaborate with nurses, therapists, and care teams to coordinate comprehensive SNF care
• Educate patients and caregivers on home programs, fall prevention, and safe mobility strategies
• Support discharge planning and smooth transitions to the next level of care
• Participate in restorative and maintenance programs to help residents maintain function

Qualifications

• Licensed Physical Therapist (PT) in Virginia, or ability to obtain state licensure prior to start
• DPT degree preferred; candidates with required PT credentials are encouraged to apply
• New grads welcome — the team values enthusiasm and is ready to mentor
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfort working in a Skilled Nursing Facility (SNF) environment and collaborating across disciplines
• Strong communication, documentation, and time-management skills

Education

• DPT (Doctor of Physical Therapy) preferred; PT degree required for licensure
• New graduates with appropriate degree and active/candidate licensure are encouraged to apply

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, transfers, and ADL-focused rehab
• Familiarity with SNF workflows, restorative care, and interdisciplinary care planning
• Competent clinical documentation and ability to meet regulatory/payer requirements
• Patient-centered approach with excellent interpersonal and caregiver education skills
• Adaptable clinical reasoning to serve a diverse caseload, including short-term and long-term residents
