Roles & Responsibilities

• Assess patients' functional mobility and develop individualized treatment plans
• Implement therapeutic interventions to improve strength, balance and independence
• Collaborate with nursing and interdisciplinary teams to coordinate patient care
• Document progress and outcomes accurately and timely in the patient record
• Educate patients and families on exercises, safety and discharge planning
• Participate in orientation and onboarding activities to learn facility protocols

Qualifications

• Active Physical Therapist license in California or eligibility to obtain licensure
• Current BLS/CPR certification or willingness to obtain during orientation
• Experience working in inpatient, skilled nursing or rehabilitation settings preferred
• Strong communication skills and ability to collaborate with multidisciplinary teams
• Reliable professional references and ability to pass facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Completion of accredited physical therapy program required

Knowledge & Skills

• Clinical expertise in mobility, balance and gait training
• Skilled in therapeutic exercise progression and adaptive equipment recommendations
• Proficient in documentation and outcome measurement tools
• Ability to prioritize caseloads and manage time efficiently
• Compassionate patient education and family engagement skills
