Roles & Responsibilities

• Evaluate patients and create individualized treatment plans to restore mobility and function
• Deliver hands-on therapy and therapeutic exercises to promote recovery in a rehab/skilled nursing setting
• Collaborate with nurses, OT, SLP, and interdisciplinary team members to coordinate care
• Monitor patient progress, update goals, and adjust plans to maximize outcomes
• Maintain accurate documentation and communicate clearly with families and caregivers
• Support safety and quality initiatives to enhance the patient experience

Qualifications

• Licensed Physical Therapist in Texas or eligible for Texas licensure
• Experience in rehabilitation or skilled nursing settings preferred
• Current CPR/BLS certification (or willingness to obtain)
• Strong clinical assessment and treatment planning skills
• Excellent communication and teamwork abilities

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor of Science in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in therapeutic exercise and manual therapy techniques
• Knowledge of mobility, gait training, transfers, and ADL retraining
• Familiarity with rehab documentation and regulatory requirements
• Ability to collaborate effectively with interdisciplinary teams
• Good clinical reasoning and outcome measurement usage
• Comfort working in a fast-paced skilled nursing/rehab environment
