Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and needs
• Design and implement individualized treatment plans that promote independence
• Deliver therapeutic interventions to improve strength, mobility, and function
• Educate patients and families on exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, and physicians to coordinate care
• Monitor patient progress and adjust treatment plans to achieve the best outcomes
• Maintain accurate and timely clinical documentation in accordance with facility policies
• Ensure care plans comply with regulatory requirements and best practices

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid California Physical Therapy license
• Experience in a skilled nursing facility or similar setting preferred
• Strong knowledge of therapeutic modalities, adaptive equipment, and treatment planning
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of an interdisciplinary care team
• Compassionate, patient-centered approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
