Roles & Responsibilities

• Assess patients' movement dysfunction and pain to create personalized treatment plans that improve daily function and mobility
• Deliver hands-on and therapeutic interventions focused on pain management, fall risk reduction, and mobility improvement
• Plan and support discharge processes to help patients transition safely back to home or next level of care
• Document evaluations, progress notes, daily notes, discharges, and any communications with insurers or providers accurately and timely
• Communicate clearly with patients, families, providers, rehab staff, supervisors, and insurance companies to coordinate care
• Support clinic operations by assisting with light cleaning tasks, treatment room upkeep, and laundry as needed
• Perform other duties as requested to contribute to a safe, efficient, and patient-centered rehab environment

Qualifications

• Valid Physical Therapy licensure as required by the state (applicable credential required prior to patient care)
• Demonstrated experience or competency in pain management, fall risk reduction, mobility improvement, and discharge planning
• Strong patient evaluation and documentation skills, including progress notes and insurance-related documentation
• Excellent verbal and written communication skills for interacting with patients, families, providers, and insurers
• Ability and willingness to assist with basic clinic upkeep tasks (cleaning, laundry) when needed
• Comfort working full-time day shifts in a fast-paced rehab setting

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Bachelor’s degree in a related field acceptable with appropriate licensure
