Roles & Responsibilities

• Carry out individualized treatment plans under the direct supervision of a Physical Therapist, documenting each visit thoroughly
• Assist in developing plans of care with Physical Therapists to promote functional independence and safety
• Provide skilled restorative and maintenance therapeutic techniques and patient/caregiver education during visits
• Assess patient status and progress on every visit and ensure quality of care is reflected in the clinical record
• Coordinate patient care with physicians, nurses, caregivers, and other rehab team members to ensure continuity and clear communication
• Remain vigilant for changes in patient condition and promptly report status changes to the case manager
• Communicate ongoing clinical and operational activities with supervising Physical Therapists to maximize outcomes
• Complete required documentation and compliance paperwork in line with federal/state regulations and organizational policies

Qualifications

• Demonstrated ability to perform PTA duties under the direct supervision of a Physical Therapist
• Strong clinical documentation skills and familiarity with regulatory compliance
• Knowledge of and ability to follow APTA guidelines and HHCAHPS standards
• Proven ability to educate patients and caregivers and implement treatment plans
• Excellent communication and care coordination skills with interdisciplinary teams
• Comfort reporting changes in patient condition and escalating to case management in a timely fashion
