Roles & Responsibilities

• Assess patients' physical function and mobility using evidence-based evaluation techniques
• Design and implement individualized treatment plans to improve mobility, reduce pain, and restore function
• Provide hands-on skilled care including gait training, post-op rehabilitation, and neurological rehabilitation
• Document thoroughly and timely in Epic to support continuity of care and billing requirements
• Collaborate with interdisciplinary teams to coordinate discharge planning and ensure safe transitions
• Educate patients and families on home exercise programs and strategies for long-term recovery
• Use safe handling equipment (gait belts, sliding boards, specialty beds) to protect patients and staff

Qualifications

• Active Physical Therapist license (required)
• Minimum 1 year of experience in Acute Care physical therapy
• Experience with Epic charting system (required)
• BLS certification preferred
• Comfort working a 1:1 patient ratio in an acute care setting
• Strong communication skills for interdisciplinary collaboration and patient/family education

Knowledge & Skills

• Acute care assessment, documentation, evaluation, and discharge planning
• Gait training, transfers, and safe patient handling
• Orthopedics and neurologic rehabilitation (post-op hip/knee, stroke, spinal cord, TBI)
• Familiarity with Medicare A, Medicaid, and commercial billing basics
• Use of equipment and devices: gait belts, sliding boards, hospital/specialty beds, bed alarms
• Home exercise program design and patient/family education
