Roles & Responsibilities

• Evaluate residents and conduct thorough patient assessments to identify strengths and needs
• Develop individualized treatment plans focused on improving function, ADLs, and safety
• Provide evidence-based therapeutic interventions to restore independence and quality of life
• Collaborate closely with nursing and rehabilitation staff to coordinate care and optimize outcomes
• Maintain accurate, timely documentation in compliance with state and federal regulations
• Contribute to a positive, resident-centered environment through compassionate care and teamwork

Qualifications

• Current California Occupational Therapist (OT) license (required)
• Strong interpersonal and clinical skills with a commitment to quality patient outcomes
• Experience in skilled nursing or post-acute care preferred but not required
• Comfortable with documentation and regulatory compliance expectations
• Enthusiasm for mentorship, growth, and working within an interdisciplinary team

Knowledge & Skills

• Proficiency in treatment planning and therapeutic interventions
• Skilled in Activities of Daily Living (ADLs) assessment and rehabilitation
• Strong patient evaluation and clinical problem-solving abilities
• Experience with documentation and meeting state/federal compliance requirements
• Ability to collaborate effectively with nursing, rehab staff, and leadership in a post-acute setting
