Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care including vital signs, ADLs, and routine assessments
Administer medications and treatments per physician orders and facility protocols
Document patient care accurately and timely in the chart to ensure continuity
Communicate effectively with RNs, physicians, patients, and families to support care plans
Assist with admissions, discharges and transfers to keep patient flow smooth
Monitor patients for changes in condition and promptly report concerns to the care team

Qualifications

Current LPN license in Pennsylvania (or compact/eligible where applicable)
Current BLS/CPR certification (or willingness to obtain upon hire)
Valid immunizations and required health screenings per facility policy
Professional, patient-focused attitude and strong teamwork skills

Education

Graduate of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

Solid clinical assessment and basic nursing skills appropriate for LPN practice
Medication administration knowledge and safe practice
Clear, compassionate communication with patients and interdisciplinary teams
Accurate documentation and attention to detail
Ability to prioritize tasks in a fast-paced healthcare setting
