Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents as an LPN
• Administer medications and treatments accurately and document thoroughly
• Monitor patient conditions and communicate changes to the care team
• Support admissions, discharges, and transfers with clear, calm coordination
• Collaborate with RNs, physicians, and multidisciplinary staff to optimize care plans
• Educate residents and families about care plans, medications, and safety
• Maintain regulatory and facility documentation with attention to detail

Qualifications

• Current and active LPN license (state licensure required)
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Prior experience in long-term care or skilled nursing is a plus
• Ability to pass background check, drug screen, and health screenings
• Strong communication skills and a team-first attitude

Education

• Practical Nursing diploma or equivalent completed from an accredited program

Knowledge & Skills

• Solid clinical assessment and medication administration skills
• Familiarity with documentation standards and electronic charting
• Ability to prioritize tasks in a fast-paced environment
• Compassionate patient-centered care and strong interpersonal skills
• Infection control practices and safety-focused mindset
