Roles & Responsibilities

• Provide compassionate direct patient care, ensuring dignity and comfort for each resident
• Administer medications and treatments accurately and safely according to orders
• Monitor and record vital signs and communicate changes in patient condition promptly
• Document patient status and care provided in the medical record clearly and timely
• Collaborate with RNs, CNAs, and interdisciplinary team members to coordinate care plans
• Assist with admissions, transfers, and discharges to ensure smooth transitions
• Educate residents and their families on care plans, medications, and health maintenance

Qualifications

• Current, active LPN license in Pennsylvania (required)
• CPR/BLS certification (current) or willingness to obtain
• Proof of required immunizations and ability to pass background and drug screening
• Strong communication and interpersonal skills; compassionate bedside manner
• Comfortable working in a long-term care/skilled nursing environment; prior LPN experience is a plus

Education

• LPN diploma or vocational nursing certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and pressure injury prevention
• Patient assessment skills and recognizing changes in condition
• Familiarity with infection control procedures
• Electronic charting and accurate recordkeeping
• Teamwork and clear communication with interdisciplinary staff
