Roles & Responsibilities

• Provide compassionate direct patient and resident care with attention to dignity and comfort
• Administer medications and treatments safely and accurately
• Perform timely assessments and document changes in condition in the chart
• Collaborate with RNs and the interdisciplinary team to support care plans
• Monitor vital signs, report concerns, and escalate when needed
• Assist with activities of daily living while promoting independence
• Maintain infection control practices and safety standards
• Support family communication and contribute to a positive patient experience

Qualifications

• Active LPN license in Pennsylvania (required)
• Graduate of an accredited Practical Nursing program or equivalent
• Current BLS/CPR certification (or willingness to obtain)
• Comfortable with medication administration and basic nursing procedures
• Strong communication skills and ability to work well on a care team
• Reliable, professional, and able to pass background and health screenings

Education

• Practical Nursing diploma or LPN certificate from an accredited program

Knowledge & Skills

• Patient assessment and basic clinical evaluation
• Medication administration and documentation
• Wound care and restorative nursing support
• Electronic charting / EHR documentation skills
• Infection control and safety practices
• Time management and prioritization in a busy care setting
• Effective communication with patients, families, and staff
• Teamwork and collaboration across disciplines
