Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on time
Perform routine assessments and report changes in condition promptly
Document care thoroughly and maintain clear communication with the care team
Assist with activities of daily living to promote dignity and independence
Follow infection control policies and facility procedures to keep everyone safe
Collaborate with nurses, therapists, and support staff to deliver coordinated care

Qualifications

Active Licensed Practical Nurse (LPN) credential — Pennsylvania license preferred
Recent clinical experience as an LPN in long-term care, skilled nursing, or similar setting is a plus
Comfortable administering medications and performing routine assessments
Strong communication skills and a patient-first attitude
Ability to pass required background checks and facility onboarding

Education

Completion of an accredited Practical Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical assessments
Care planning and clear, timely documentation
Infection control principles and safe patient handling
Team collaboration and effective communication with patients and families
Familiarity with electronic charting or willingness to learn
