Roles & Responsibilities

• Provide compassionate, resident-centered nursing care with attention to dignity and comfort
• Administer medications and treatments accurately and document appropriately
• Monitor resident condition, record vital signs, and promptly report changes
• Maintain clear, timely charting in the resident record and communicate with the care team
• Support daily living activities and collaborate with CNAs, RNs, and interdisciplinary staff
• Follow infection control and safety protocols to protect residents and coworkers
• Contribute to a positive care environment through mentorship and teamwork

Qualifications

• Active LPN (Licensed Practical Nurse) license in Pennsylvania
• Graduate of an accredited practical nursing program
• Current CPR/BLS certification (or willingness to obtain)
• Current immunizations and ability to pass background checks as required
• Strong communication skills, empathy, and ability to work well in a team
• Comfortable with medication administration, charting, and hands-on resident care

Education

• Graduate of an accredited Practical Nursing program (LPN diploma/certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Performing and interpreting vital signs and basic assessments
• Familiarity with electronic health records and accurate charting
• Infection control procedures and adherence to safety standards
• Time management and prioritization in a busy clinical setting
• Collaborative problem-solving with interdisciplinary teams
