Location: Smethport, PA

Start Date: 04-21-2026

Contract Type: Per-Diem

Shift Type: 8-hour shift, specific start/end times and days not specified (schedule to be confirmed with recruiter)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide direct, compassionate patient care consistent with facility policies
Administer medications and treatments as prescribed, accurately and safely
Monitor and document patient condition changes and report to RN/MD promptly
Assist with activities of daily living while promoting patient dignity and independence
Collaborate with the interdisciplinary team to support care plans and positive outcomes
Maintain clear, timely charting and follow infection control protocols

Qualifications

Current Licensed Practical Nurse (LPN) license (Pennsylvania preferred) or eligible to obtain state licensure
Basic Life Support (BLS) certification required or willing to obtain
Valid immunizations and ability to pass background and drug screening
Excellent communication skills and a patient-first attitude
Previous LPN experience in a long-term care or similar setting is a plus

Education

Completion of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Strong clinical assessment and basic nursing care skills
Medication administration and documentation accuracy
Familiarity with electronic health records and charting best practices
Infection control and patient safety procedures
Ability to prioritize tasks in a busy clinical environment
Teamwork and effective communication with patients, families and staff
