Roles & Responsibilities

Provide compassionate direct patient care and support daily living activities
Administer medications and treatments per provider orders and facility policy
Monitor, assess and document patient status accurately and timely
Collaborate with RNs and the interdisciplinary care team to deliver excellent care
Maintain patient safety and infection control standards at all times
Communicate clearly and kindly with patients, families and staff to ensure continuity of care

Qualifications

Licensed Practical Nurse (LPN) certification/license (active or eligible for licensure in Pennsylvania)
Current Basic Life Support (BLS) / CPR certification preferred
Excellent communication and teamwork skills
Comfortable with electronic charting and standard clinical documentation
Prior experience in long-term care, skilled nursing, or acute care is a plus

Education

Completion of an accredited Practical Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital signs monitoring
Wound care fundamentals and infection prevention
Accurate clinical documentation and charting
Time management and prioritization in a busy clinical setting
Strong interpersonal skills for patient and family interactions
