Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care while following care plans and facility policies
Administer medications and treatments accurately and safely
Assess patient status and report changes promptly to the care team
Document care thoroughly in the medical record with attention to detail
Collaborate with nurses, therapists, and physicians to coordinate excellent care
Support residents' daily living activities and promote dignity and independence
Participate in quality improvement initiatives and safety practices
Build trusting relationships with patients and families through clear, empathetic communication

Qualifications

Current LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program
Current CPR/BLS certification or willingness to obtain promptly
Comfortable with medication administration, wound care basics, and charting
Strong communication skills and a team-oriented attitude
Previous long-term care or skilled nursing experience is a plus

Education

Graduate of an accredited practical nursing program (LPN/LVN diploma or equivalent)

Knowledge & Skills

Medication administration and safe documentation practices
Basic patient assessment and vital signs interpretation
Familiarity with electronic health records and charting
Infection control standards and universal precautions
Time management, prioritization, and delegation within scope
Empathy, de-escalation techniques, and effective patient communication
