Roles & Responsibilities

Provide compassionate direct patient care following established care plans
Administer medications and treatments as ordered and document accurately
Monitor patient conditions and report changes to the interdisciplinary team
Assist with activities of daily living to promote patient comfort and dignity
Collaborate with nurses and other clinicians to deliver safe, high-quality care
Maintain accurate, timely clinical documentation and follow facility policies

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact-state equivalent
Valid BLS/CPR certification or willingness to obtain upon hire
Comfortable working an 8-hour clinical shift and providing hands-on care
Strong communication skills and ability to work collaboratively in a team
Reliable, patient-centered mindset with attention to detail and safety

Education

Completion of an accredited practical nursing program or vocational nursing diploma

Knowledge & Skills

Practical nursing skills including medication administration and wound care
Basic assessment skills and ability to recognize changes in patient condition
Charting and documentation best practices
Ability to follow care plans and facility protocols
Good interpersonal skills for patient, family and team interactions
