Roles & Responsibilities
- Provide compassionate direct care to residents including activities of daily living
- Administer medications and treatments safely and accurately
- Monitor resident condition and report changes to the care team promptly
- Document care in the electronic health record clearly and timely
- Collaborate with nurses, therapists and families to develop and follow care plans
- Support a positive, resident-centered environment and promote dignity
Qualifications
- Active LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program or equivalent
- Current CPR/BLS certification required or must obtain upon hire
- Prior experience in long-term care, skilled nursing or related setting preferred
- Strong communication skills and a team-oriented mindset
Education
- Practical Nursing diploma or equivalent
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic clinical assessments
- Wound care and infection control practices
- Familiarity with electronic health record documentation
- Ability to prioritize tasks in a fast-paced care environment
- Compassionate resident-centered care and strong interpersonal skills
