Roles & Responsibilities

Provide compassionate, patient-centered nursing care as an LPN
Administer medications and document patient responses accurately
Collaborate with RNs and interdisciplinary team members to support care plans
Monitor patients and report changes in condition promptly and clearly
Assist with admissions, discharges, and transitions of care with professionalism
Maintain accurate charts and complete required documentation on time
Support a positive environment for residents and families through respectful communication

Qualifications

Current LPN license (PA preferred) or ability to obtain state licensure
CPR/BLS certification or willingness to obtain upon hire
Ability to pass required background checks and immunization verification
Comfortable working in a long-term care/clinical setting and collaborating with a care team
Strong communication skills and a patient-first attitude

Education

Practical Nursing diploma, vocational certificate, or equivalent LPN program completion

Knowledge & Skills

Fundamental nursing assessment and basic medication administration
Familiarity with charting and documentation best practices
Basic infection control and safety procedures
Ability to communicate clearly with patients, families, and interdisciplinary staff
Time management and prioritization for short-shift workflows
