Contract Type: Per-Diem

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct nursing care to residents, focusing on dignity and comfort
Administer medications and treatments safely and accurately
Monitor and document vital signs, observations, and changes in condition
Collaborate with the interdisciplinary team to develop and follow care plans
Assist residents with activities of daily living while promoting independence
Maintain accurate clinical documentation and communicate effectively with families
Follow infection control and safety protocols to protect residents and staff
Participate in unit shifts and handoffs to ensure continuity of care

Qualifications

Current and active LPN license (Licensed Practical Nurse) — Pennsylvania licensure preferred
Completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification — current
Strong medication administration and charting skills
Compassionate bedside manner and good communication skills
Previous long-term care or skilled nursing experience preferred

Education

Completion of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Clinical assessment and basic nursing procedures
Medication administration and documentation
Knowledge of infection control and safety standards
Ability to work within an interdisciplinary care team
Time management and prioritization in a busy care setting
Electronic health record (EHR) familiarity is a plus
