Roles & Responsibilities

• Deliver compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments safely and accurately according to facility policies
• Monitor residents' health status and report changes promptly to the care team
• Complete clinical documentation, including MDS-related tasks if applicable
• Collaborate with RNs, CNAs, families, and interdisciplinary staff to develop care plans
• Support quality improvement efforts and maintain regulatory compliance
• Promote a positive environment that respects resident dignity and independence

Qualifications

• Current, unrestricted Colorado LPN license (required)
• Licensed Practical Nurse (LPN) credential
• Long-term care (LTC) experience (required)
• MDS experience (preferred)
• Ability to start by the assignment start date and work the full contract

Education

• Completion of an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Knowledge of long-term care practices and resident-centered approaches
• Familiarity with MDS processes and documentation (preferred)
• Good communication and teamwork skills to work with residents and interdisciplinary staff
• Organizational skills for accurate charting and timely follow-up
