Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in long-term care
• Administer medications and treatments safely and accurately
• Assess resident conditions and document changes in status
• Follow individualized care plans and communicate updates to the team
• Assist with activities of daily living while promoting resident dignity
• Perform routine clinical tasks such as vital signs, wound care, and catheter care
• Collaborate with RNs, CNAs, and interdisciplinary staff to deliver excellent care
• Maintain clear, timely charting and comply with facility policies and regulations

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Completion of an accredited practical nursing program or equivalent
• Current Basic Life Support (BLS/CPR) certification or willingness to obtain
• Up-to-date immunizations and TB screening per facility policy
• Strong communication skills and a compassionate bedside manner
• Previous experience in long-term care or skilled nursing is a plus

Education

• Graduate of an accredited Practical Nursing (LPN/Vocational) program

Knowledge & Skills

• Medication administration and documentation best practices
• Familiarity with electronic health records and charting
• Clinical assessment skills including vital signs and wound observation
• Infection control and safe patient handling techniques
• Ability to prioritize tasks in a fast-paced care environment
• Team-oriented mindset with solid interpersonal skills
