Roles & Responsibilities
- Administer medications and treatments accurately and safely according to plan
- Assess and monitor residents’ conditions, reporting changes promptly
- Document care, observations, and resident responses clearly in the chart
- Assist residents with activities of daily living with dignity and compassion
- Collaborate with the interdisciplinary team to support individualized care plans
- Provide wound care, restorative nursing, and other delegated nursing tasks
- Support admission, transfer, and discharge processes to ensure smooth transitions
Qualifications
- Active LPN license (Licensed Practical Nurse) in Massachusetts
- Or active RN license if applying as a Registered Nurse
- Current BLS/CPR certification (or ability to obtain upon hire)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong communication skills and a compassionate approach to resident care
Education
- LPN diploma or certificate for Licensed Practical Nurse positions
- Associate Degree in Nursing (ADN) or BSN for Registered Nurse applicants (if applying as RN)
Knowledge & Skills
- Medication administration and accurate documentation
- Basic wound care and restorative nursing principles
- Familiarity with long-term care/rehab workflows and resident assessment
- Comfort using electronic medical records and charting systems
- Excellent teamwork, time management, and resident-centered care skills
