Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents using best-practice LPN skills
Administer medications accurately and document administration in the chart
Perform routine assessments, monitor vital signs, and report changes promptly
Collaborate with the interdisciplinary team to support individualized care plans
Maintain clear, timely documentation and adherence to facility policies
Support resident comfort and dignity through respectful communication and care
Participate in shift handoffs to ensure continuity and safety of care

Qualifications

Current, active LPN license in Pennsylvania
Graduate of an accredited practical nursing program
CPR/BLS certification preferred or willingness to obtain
Ability to pass background checks and required health screenings
Strong communication skills and a positive, team-oriented attitude
Dependable, punctual, and committed to high standards of patient care

Education

Graduate of an accredited practical nursing program
Current LPN licensure in Pennsylvania

Knowledge & Skills

Medication administration and accurate charting
Basic patient assessment and recognition of changes in condition
Infection control practices and safe clinical technique
Ability to work with care plans and follow nursing orders
Good interpersonal skills for interacting with residents and families
Familiarity with electronic health records or willingness to learn
