Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and timely
Perform routine assessments and monitor patient status
Document patient information clearly in the medical record
Collaborate with the care team to develop and implement care plans
Communicate effectively with patients, families, and interdisciplinary staff
Assist with activities of daily living and promote patient comfort and safety

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Completion of an accredited practical nursing program (diploma or certificate)
Strong attention to detail and solid clinical judgment
Professional communication skills and a team-oriented attitude
Previous nursing experience in a long-term care or similar setting preferred

Education

Diploma or Certificate in Practical Nursing from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Patient assessment and monitoring techniques
Electronic medical record (EMR) charting familiarity
Infection control and safety procedures
Time management and prioritization in a busy clinical setting
Compassionate bedside manner and effective interpersonal skills
