Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents/patients with professionalism
• Administer medications and treatments accurately and document per facility protocols
• Monitor patient status and report changes promptly to the interdisciplinary team
• Assist with ADLs (activities of daily living) while promoting dignity and independence
• Collaborate with nurses and care staff to support high-quality, person-centered care
• Maintain clear, timely charting and follow all regulatory and facility documentation standards

Qualifications

• Active LPN license in Pennsylvania (or eligible to obtain one) — required
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Previous LPN experience in a long-term care, skilled nursing, or similar setting preferred
• Strong communication skills and a team-first attitude
• Ability to pass required background checks and workplace health screenings

Education

• Completed Practical Nursing program or equivalent LPN/LVN diploma or certificate

Knowledge & Skills

• Fundamental nursing skills including medication administration and wound care
• Comfortable with resident-focused assessments and basic charting
• Good clinical judgment and ability to escalate concerns appropriately
• Strong interpersonal skills for working with patients, families, and care teams
• Time management and organizational skills to handle shift responsibilities efficiently
