Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide direct, compassionate patient care including vital signs and personal care
Administer medications and treatments accurately and document appropriately
Monitor patient condition and report changes to the RN or provider promptly
Perform wound care and routine clinical procedures per facility protocols
Maintain clear, timely charting in the electronic health record
Collaborate with the interdisciplinary team to support individualized care plans
Educate patients and families on care plans, medications, and safety precautions

Qualifications

Active LPN license in Pennsylvania (must be in good standing)
Completion of an accredited practical nursing program or equivalent diploma/certificate
BLS/CPR certification required or must obtain upon hire
Current immunizations and ability to pass background check and drug screen
Comfortable working in a long-term care or skilled nursing environment (experience preferred)

Education

Diploma or certificate from an accredited Practical Nursing program (LPN)

Knowledge & Skills

Strong medication administration and documentation skills
Knowledge of infection control and patient safety practices
Familiarity with charting in electronic health record systems
Excellent communication and teamwork abilities
Ability to prioritize tasks in a fast-paced clinical setting
Compassionate bedside manner with attention to detail
