Start Date: 04-07-2026

Contract Type: Per-Diem

Shift Type: 8-hour shift (start/end times and days to be confirmed by facility)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents consistent with care plans
Administer medications and treatments accurately and safely
Document resident conditions, interventions, and outcomes clearly and promptly
Collaborate with the care team to ensure continuity and quality of care
Observe and report changes in resident status to RN/supervisor in a timely manner
Support residents with daily living activities while promoting dignity and independence

Qualifications

Current LPN license (eligible to practice in Pennsylvania)
Graduate of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willingness to obtain
Ability to pass required background checks and health screenings
Strong communication skills and a resident-centered approach
Previous long-term care or skilled nursing experience preferred

Education

Graduate of an accredited Practical Nursing program (LPN/LVN)

Knowledge & Skills

Skilled in medication administration and documentation
Comfortable with ADLs, wound care basics, and vital signs monitoring
Familiarity with electronic charting or willingness to learn facility EMR
Ability to prioritize tasks and work collaboratively in a fast-paced setting
Strong observational skills and clinical judgement
