Roles & Responsibilities

• Provide compassionate direct nursing care to residents in accordance with facility policies
• Administer medications and treatments safely and accurately per provider orders
• Perform timely head-to-toe assessments and document patient status in the chart
• Collaborate with RNs, physicians, therapists and support staff to implement care plans
• Educate patients and families about care needs, medications and discharge instructions
• Maintain a safe, clean environment and follow infection control best practices
• Participate in meaningful shift handoffs and communicate changes in patient condition

Qualifications

• Current LPN licensure — New Hampshire license preferred
• Basic Life Support (BLS/CPR) certification required or obtained upon hire
• Prior LPN experience in a long-term care or skilled nursing setting preferred
• Current immunizations and ability to pass required background and health screenings
• Excellent communication skills and a team-oriented mindset

Education

• Graduate of an accredited practical nursing program (certificate or diploma)

Knowledge & Skills

• Medication administration and accurate documentation
• Skilled with patient assessments and recognizing changes in condition
• Familiarity with electronic health record documentation
• Strong clinical judgment, time management, and prioritization
• Good interpersonal skills for working with patients, families and care teams
