Roles & Responsibilities

• Provide safe, compassionate nursing care to patients on the unit during the evening shift
• Administer medications and treatments accurately and document per facility policy
• Perform thorough patient assessments and communicate changes to the interdisciplinary team
• Coordinate care transitions and contribute to patient-centered discharge planning
• Support and mentor less-experienced staff when appropriate
• Maintain accurate and timely charting in the electronic medical record

Qualifications

• Current LPN license (Licensed Practical Nurse) in good standing — NH licensure or compact eligibility preferred
• Previous hands-on nursing experience in an acute or long-term care setting is a plus
• Basic Life Support (BLS/CPR) certification — obtain or maintain as required by facility
• Ability to work the evening shift (3:00 PM - 11:30 PM) and every-other-weekend rotation
• Strong communication skills and a collaborative mindset
• Reliable clinical skills in medication administration, assessments, and documentation

Education

• LPN diploma or equivalent practical nursing program completion
• High school diploma or GED

Knowledge & Skills

• Comfortable with electronic health record (EHR) documentation
• Skilled in clinical assessment and prioritization during busy evening hours
• Knowledge of infection control standards and patient safety practices
• Ability to collaborate with interdisciplinary teams including RNs, providers, and therapists
• Excellent time management and the ability to multitask in a fast-paced environment
• Customer-service oriented approach to patient and family interactions
