Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term/rehab setting
• Administer medications and treatments accurately and safely
• Assess residents’ conditions and promptly report changes to the care team
• Document care thoroughly in the medical record to ensure continuity and quality
• Collaborate with RNs, therapists and support staff to create and follow individualized care plans
• Support admissions, discharges and transitions with professionalism and empathy
• Promote a safe environment through infection control, fall prevention and best practices

Qualifications

• Current, active LPN license (eligible to work in Massachusetts)
• Open to Registered Nurses as well (RN credential also accepted)
• BLS/CPR certification required or must be able to obtain promptly
• Comfortable working in a skilled nursing/rehabilitation setting; prior long-term care experience is a plus
• Strong communication skills and ability to work well within a multidisciplinary team

Education

• Graduate of an accredited practical nursing program (LPN) or accredited nursing program for RNs (ADN/BSN)

Knowledge & Skills

• Solid fundamentals in patient assessment, vital signs and basic nursing procedures
• Medication administration and accurate documentation skills
• Familiarity with care planning and charting in an electronic health record
• Good clinical judgment and the ability to escalate concerns appropriately
• Compassionate resident-centered care and strong interpersonal skills
