Roles & Responsibilities

• Provide skilled physical therapy services under the supervision of a licensed PT across outpatient, inpatient, swing bed, and home health settings
• Manage an active caseload effectively and compassionately, prioritizing patient safety and outcomes
• Collaborate with the interdisciplinary team to create and implement individualized treatment plans
• Document patient progress accurately and timely in the electronic medical record
• Educate patients and families on mobility, transfers, exercises, and injury prevention
• Demonstrate cultural sensitivity and, when available, use Spanish language skills to enhance patient communication
• Participate in team meetings, discharge planning, and continuity of care across settings

Qualifications

• Current LPTA license in good standing (required)
• CPR certification (required)
• Comfortable using computers and electronic medical records
• Strong interpersonal skills and a proven team-player attitude
• Spanish language skills are a plus
• Able to work Monday–Friday daytime schedule

Knowledge & Skills

• Outpatient care and treatment planning
• Inpatient and swing bed rehabilitation processes
• Home health evaluation and safe discharge practices
• Strong documentation and EMR navigation skills
• Effective patient education and communication skills
• Teamwork and interdisciplinary collaboration
• Basic proficiency in Spanish (preferred)
• CPR and safety-focused clinical practice
